Participant waiver

1. Assumption of Risk (California Youth Sports)

I, the undersigned parent or legal guardian of the above-named minor participant, understand that participation in youth soccer and related activities involves inherent risks, including but not limited to physical injury, concussion, illness, permanent disability, paralysis, or death.

These risks may result from:

• Physical contact with other participants
• Falls, collisions, or overexertion
• Field conditions, weather, or equipment
• Actions or inactions of coaches, referees, volunteers, spectators, or other participants

I knowingly, voluntarily, and expressly assume all such risks, both known and unknown, on behalf of myself and the participant, even if arising from the negligence of the released parties, to the fullest extent permitted under California law.

2. Release and Waiver of Liability

On behalf of myself and the participant, I hereby release, waive, discharge, and hold harmless Kern County Kits, including its directors, officers, coaches, assistant coaches, volunteers, employees, referees, sponsors, event organizers, facility owners, and affiliated organizations, from any and all claims, demands, damages, losses, or causes of action arising out of or related to participation in Kern County Kits activities, including travel to and from events.

This release includes claims based on ordinary negligence, to the fullest extent allowed by the laws of the State of California.

3. Concussion & Head Injury Acknowledgement (California Compliance)

I understand that concussions and other traumatic brain injuries are serious injuries that can occur in soccer and may result from impacts to the head, body, or ground.

I acknowledge and agree that:

• I have been informed of the signs and symptoms of concussion, which may include headache, dizziness, confusion, nausea, sensitivity to light or noise, memory problems, or loss of consciousness.
• If a concussion or suspected head injury occurs, the participant must be removed from play immediately.
• The participant may not return to play until evaluated and cleared in writing by a licensed healthcare provider trained in concussion management, in accordance with California youth sports guidelines.
• I am responsible for monitoring the participant at home and seeking medical attention if symptoms appear or worsen.

I understand and accept that failure to follow concussion protocols may result in removal from league activities.

4. Medical Authorization

In the event of an accident, injury, or medical emergency, I authorize Kern County Kits and its representatives to obtain emergency medical treatment for the participant if I cannot be reached.

I understand and agree that I am financially responsible for all medical expenses incurred.

5. Code of Conduct & Safety Compliance

I agree that the participant will comply with all league rules, safety policies, and coach instructions. I understand that unsafe behavior or failure to follow rules may result in disciplinary action or removal from activities without refund.

6. Photo & Video Release

I grant permission for Kern County Kits to use photographs and/or video recordings of the participant for league-related promotional, educational, or informational purposes without compensation. (You choose Yes or No for each player on the registration form.)

7. Governing Law & Severability

This agreement shall be governed by and interpreted in accordance with the laws of the State of California. If any portion of this agreement is found unenforceable, the remaining provisions shall remain in full force and effect.

8. Acknowledgement & Signature

I certify that I am the parent or legal guardian of the participant. I have read, understood, and voluntarily agree to all terms of this Release of Liability, Waiver of Claims, Assumption of Risk, and Concussion Acknowledgment.
